We welcome all new and returning patients to our practice. If you would like to attend our practice, you can come visit the surgery at any time from 9am to 5pm Monday to Friday.
There are a few ways in which you can register with us;

  • Our reception staff can take your registration details when you walk in (please bring your ID)
  • Phone reception on 021 4821280 for a basic setup before you arrive
  • Send a registration request through our online service, we can then call you back to activate a new patient file on our system

Please note a valid identification (Driving Licence or Passport) will be required when you present yourself at reception to complete full registration on our system.

A Change of doctor form will be given to you to sign. Once you have been accepted as a patient, your medical records will be transferred to the practice and you will be sent a new medical card. Your medical records may take up to several weeks to arrive at the practice. If you have a preference with regards to registering with a particular doctor, please make this known to the receptionist when registering.

The practice has a non-discrimination policy for accepting new patients. There are a number of reasons why you may not be able to register with your chosen GP. For example, you may live too far away. If this is the case, simply choose another GP in your local area.

If English is not your first language, the practice has access to interpreters. Please inform reception staff and this assistance can be provided.

For Patients who require a Medical Card, please click on the the application form below. This should be filled out and returned to our reception staff
